Complex Primary Total Knee Replacement Surgery

WHAT IS COMPLEX PRIMARY TOTAL KNEE REPLACEMENT SURGERY?

A complex knee replacement may be needed if you’re having a second or third joint replacement in the same knee, or if your arthritis is very severe.

Some people may need a more complex type of knee replacement. The usual reasons for this are:

major bone loss due to arthritis or fracture
major deformity of the knee
weakness of the main knee ligaments

It’s different from routine knee replacement surgery in a way that requires additional extension rods on either femur/tibia or on both sides. It may or may not require additional osteotomy.

INDICATIONS

(1) Excessive varus deformity (Both legs bowed inwards)

More than 90% patients have varus deformity in arthritis (Both legs bowed inwards) but when its more than 15 Degrees and associated with deformity of tibia it may require extension rods in tibia.

(2) Excessive valgus deformity (both legs going outwards)

Excessive valgus deformity mostly seen in Rheumatoid arthritis. It possesses many challenges in technique and final outcome as the whole surgical principles are different and requires expertise.

(3) Stress fracture in proximal tibia

These are unique fractures in a way that doesn’t require major trauma or fall but develops over a long time because of long standing deformity.
Sometimes they are not even diagnosed on x-ray and requires MRI.
Major symptoms are severe pain and inability to walk.
They usually require proximal fibular osteotomy as well for the better healing.
Advantage of treating them with TKR is that even though with double surgery we can make patients walk from the very next day.

(4) After HTO (High Tibial Osteotomy)

With previous fracture surgeries mostly there are fracture implants are already present and altered anatomy possess challenge in TKR. Other most common knee preservation surgery HTO (High tibial osteotomy). It requires removal of implant as well as removal of lot of fibrous tissues around joint.

I met Dr Rachit initislly for my father's right knee problem, which has been troubling him for more than three years. Though my father was not ready for a replacement, Dr Rachit convinced him and also assured him about a good recovery and that he will be able normally as before.
